JOB DESCRIPTION:
Position Summary:

The transplant coordinator provides nursing care in the acute care setting to the transplant patient population. Care goals include promotion of healing and prevention of illness and recidivism. The transplant coordinator participates in the team management of patients functioning in a collaborative role with the physicians and other members of the healthcare team. The transplant coordinator facilitates the transition of patients between service units and provides/coordinates complex patient and family teaching. May need to participate in call rotation based on program need. Serves in an integral role for the comprehensive coordination of all aspects of patient care. Must possess exemplary communication and relationship building skills; and demonstration of highest standards of professional conduct. Protects the rights and individual privacy of patients, their families/legal guardians, and other health care team members.

Key Responsibilities/Essential Functions:
Other duties may be assigned. Regular and reliable attendance is an essential function of the job.
· Coordinates and facilitates the conduct and review of the designated testing required to determine a patient’s candidacy.
· Coordinates with multidisciplinary team the evaluation process for potential candidates. Functions as a candidate advocate by applying knowledge of growth, development, education and cultural background during candidate/family/legal guardian interactions.
· Collects and evaluates medical, psychosocial and financial data for review by transplant team.
· Demonstrates knowledge of, and compliance with, OPTN/UNOS policies and regulations. Acts as a resource person for internal and external health care providers.
· Provides and maintains comprehensive, accurate and thorough documentation per EMR, UNOS and medical center records.
· Maintains monitoring of patient status and follows through on facilitating the medical plan of care throughout the process of evaluation through listing/waiting period, transplantation and post-operatively.
· Communicates with referring physicians by verbal and written information.
· Assists in coordination of all aspects of post procedure care to include, but not limited to, laboratory data and all interval diagnostic studies. Evaluates and facilitates general health status and patient risk factor management on a regular basis.
· Communicates all patient issues and concerns to medical providers and other members of the multidisciplinary team members.
· Collaborates with nursing staff, social workers and other health care providers to establish and implement patient care plan according to current standards of care.
· Coordinates the care of the patient in the outpatient setting to achieve optimal physical, social, and emotional rehabilitation.
· Participates in rotating schedule for the coordination of patient care concerns and post operative patient needs, according to program needs as identified by leadership.
· Possesses exceptional communication skills, problem solving, and organizational skills as well as a commitment to the highest quality in clinical care.
· Maintains the highest standards of professional and ethical conduct.
· Plans, conducts and evaluates educational presentations workshops and seminars utilizing materials developed for professional education as requested.
· Participates in all quality and performance improvement activities.
· Maintains current knowledge of and complies with both institutional policies and procedures and all regulatory bodies.
· Maintains cooperative relationships with other health care professionals.
· Reviews and integrates into practice the current professional literature in the field of transplant. Provides for patient care guided by evidenced based practice guidelines/protocols as appropriate.
· Examines current related research. Participates in and supports ongoing clinical research within the transplant center and publishes results as appropriate.
· Participates in professional organizations.
· Requires self direction and represents the institution with demeanor consistent with the mission and objectives of the service and consistent with the Core Values.

JOB REQUIREMENTS:
Qualifications:
Education and/or Experience

· Graduate of a school of Nursing, BSN preferred. BCLS required. ABTC (American Board for Transplant Certification) to be obtained within 3-5 years of hire and maintained throughout employment. 2 years acute care nursing required, with a focus on nephrology, general surgery, or transplant (for Kidney/Pancreas Transplant program) or cardiology, CV surgery, or transplant (for Cardiac Transplant/VAD program).

Certificates, Licenses, Registrations
· Registered Nurse with current Kansas license required.
